Position Summary

We're seeking a highly proactive, compassionate, and relationship-driven Care Manager to serve as a vital connection between clients, families, caregivers, and the administrative team. In this dynamic role, you will lead prospective client follow-up and new client intake efforts while ensuring exceptional continuity and quality of care throughout the client experience.

The ideal candidate combines clinical knowledge with strong interpersonal and organizational skills, excelling at building trust with families, guiding clients through the onboarding process, and supporting caregivers in the field. You will play a central role in converting inquiries into active clients, conducting in-home assessments and intakes, developing and updating Plans of Care, and collaborating closely with scheduling and care coordination teams to ensure services are delivered with compassion, consistency, and excellence.

This is a hands-on leadership role for someone who thrives in a people-centered environment, communicates with empathy and confidence, and takes pride in making a meaningful difference in the lives of seniors and their families every day.

Key Responsibilities

  • Directly reports to and works together with the General Manager Harrison Skolnik, & David Legare, RN.

  • Primary on follow-up of Prospective Clients, answers questions, explains our services, strives to make a connection with the Prospect/Prospect's family, offers to schedule Intake. Sets appropriate documentation in WellSky for continued follow up as appropriate.

  • Conducts new Client Intakes as the primary. Explains our services in greater detail and obtains signed service agreement documents and Plan of Care (POC).

  • After Intake, enters notes regarding Intake, Care Summary, and Care Log tasks in the WellSky database. Sets a task in WellSky to notify all office personnel of new client details.

  • Calls caregivers to make an introduction to self and role(s), especially when the caregiver is new to a case. Answers questions related to services and work to establish rapport.

  • Completes on-site Supervisory visits of both Caregivers and Clients on a routine basis. Ensures that services provided are appropriate, adequate, and consistent with POC and observed/reported care needs. Completes new POC, as needed, and communicates accordingly with the Client Care Coordinators. Updates Care Log Tasks to coincide with updated POC.

  • Provides on-site training as needed with counseling, coaching and educating Caregivers and Clients or, if need is outside an individual's scope of experience, makes requests for follow-up from other staff members.

  • Works closely with Client Care Coordinators in making recommendations for scheduling.

  • Contributes to performance evaluations of caregivers/home health aides.

  • Identifies marketing opportunities associated with current clients, such as home health agencies, doctor offices, guardian companies, case managers, etc. Provide 'Coordination of Care' letters and/or follow-up visits when applicable. Documents marketing visits/encounters in WellSky; sets follow up task with referral source as appropriate.

  • Updates and maintains WellSky database to include daily schedule, prospective/current clients, client contacts, prospective referral sources, daily activity logs, and follow-up plans.

  • Maintains client confidentiality at all times and according to state and federal privacy laws (i.e. HIPAA).

  • Maintains and conforms to the policies and procedures that are in compliance with The Agency for Healthcare Administration (AHCA), the Centers for Disease Control (CDC), and Occupational Safety and Health Agency (OSHA) guidelines for safety, universal precautions and infection control.

  • Completes other tasks as business-related needs are identified and/or as assigned by Agency Administration.

Required Qualifications

  • Registered Nurse - 2+ Years of Experience

  • Administrative Healthcare - 2+ Years of Experience

  • Home Health Care - 2+ Years of Supervisory Experience

  • $26 - $34
